Ayush Naik gave Nerul the lead while Akash Banaulikar doubled the advantage. St Anthony responded with a late surge as Preston Dias reduced the deficit and Brandon Pereira equalised in the final minutes.
In the shootout, Siolim side kept their composure to complete a comeback.
